These terms govern your use of the Smith Family Massage Therapy website and booking form. By submitting a booking request, you agree to be contacted by Smith Family Massage Therapy regarding your appointment.

Appointments: booking requests submitted through this site are requests only and are confirmed directly by Jacob Smith by call or text.

Scope of service: Smith Family Massage Therapy provides massage therapy services and does not provide medical diagnosis or treatment. Clients with medical concerns should consult a physician.
